GENERAL DESCRIPTION:
This dataset contains 2014 Health Care Facility Information for the State of Florida. It is a combination of health care facilities (Abortion Clinic, Dialysis Clinic, Medical Doctor, Nursing Home, Osteopath, State Laboratory/Clinic, and Surgical Center/Walk-In Clinic) from the Florida Department of Health. The data contains selected fields denoting the name, physical address, and other facility information for health care facilities located in Florida. This data is meant to be used for planning purposes only and is not intended to represent a 100% inventory of Health Care in Florida. Health Care Facility locations that have been verified are marked with the letter V in the FLAG field. Please Note: Hospitals are no longer located in the GC_HEALTH_AUG14 layer, instead hospital locations can be found in the GC_HOSPITALS_FEB13 layer. This dataset is an update the FGDL layer GC_HEALTH_JUL09.

ABORTION CLINICS = Abortion Clinics

DIALYSIS CLINIC = Dialysis Clinic: A facility with dialysis machines used in dialysis that filters a patient's blood to remove excess water and waste products when the kidneys are damaged, dysfunctional, or missing.

MEDICAL DOCTOR = Medical Doctor

NURSING HOME = Nursing Home: A residential facility for people with chronic illness or disability, particularly older people who have mobility and eating problems. Also known as a convalescent home and long-term care facility.

OSTEOPATH = Osteopath Clinic: A facility that treats people who are sick or in pain by pushing and moving bones and muscles.

STATE LABORATORY/CLINIC = State Laboratory/Clinic

SURGICAL CENTER/WALK-IN CLINIC = Surgical Center/Walk-In Clinic: A Surgical Center is a facilities where surgical procedures not requiring an overnight hospital stay are performed. A Walk-In Clinic describes a very broad category of medical facilities only loosely defined as those that accept patients on a walk-in basis and with no appointment required. A number of healthcare service providers fall under the walk-in clinic umbrella including urgent care centers, retail clinics and even many free clinics or community health clinics.


FDOH_ID FDOH Identification Number

OWNER Entity owner of the facility.

OPERATING The responsible organization for management and operation of a facility (e.g., public, private, quasi-public).

OP_CLASS Classification of the operating entity.

FSOURCE Feature Spatial Source. For the following example GeoPlan is the feature source, the location was GeoCoded to the 2014 Parcel Centroid, it Matched and had a Score of 100. Example: GEOPLAN - GC_PARCELS_2014(M)[100]

DATASOURCE Feature Data/Tabular Source.

LAT_DD Latitude in decimal degrees.

LONG_DD Longitude in decimal degrees.

MGRS Military Grid Reference System (MGRS) Coordinate of the Facility. The MGRS is the geocoordinate standard used by NATO militaries for locating points on the earth. The MGRS provides a means to represent any location on the surface of the Earth using an alphanumeric string. Hierarchical references are based on the Universal Transverse Mercator (UTM) coordinate system. The MGRS is used for the entire earth. http://mgrs-data.org/

GCID The GeoPlan Center identification number that links this point feature layer with its corresponding polygon feature layer.

DESCRIPT Based on the field NAME.

FLAG Describes if the facility was visually verified or not.

What is Geocoding?
Geocoding is term used to describe the act of address matching.  Geocoding
is the process of finding a geographic location (x, y point) for an address
(such as street number and name, city, state, and ZIP Code) on a map.
Geocoding is based off the typical address scheme for the US, in which one
side of the street contains even house numbers while the other side of the
street contains odd house numbers.

The geocoding process uses an algorithm to find the geographic location
of addresses. First, a street segment is identified using the zip code and
street name. Next, the geographic location of the address is matched using
the building number to determine how far down the street and on which side
of the street the building is located.

Geocoding Accuracy
The locational accuracy of geocoded addresses may vary from urban to rural
areas due to the algorithm used to generate the geographic locations of
addresses. The algorithm assumes that the size of parcels are equivalent
along a road route. This assumption tends to be more consistent in urban
areas, where the size of parcels vary less than in rural areas.
Consequently, the results of geocoded addresses in urban areas are usually
more reliable than those in rural areas.

For example, the locational accuracy of rural addresses can be slightly
off because some parcels along a rural route may be 15 acres while others
may be 2.5 acres, but the geocoding algorithm assumes that the addresses
are distributed evenly along the route.

The data was created to serve as base information for use in GIS systems for
a variety of planning and analytical purposes.

A note about data scale: 

Scale is an important factor in data usage.  Certain scale datasets
are not suitable for some project, analysis, or modeling purposes.
Please be sure you are using the best available data. 

1:24000 scale datasets are recommended for projects that are at the
county level.
1:24000 data should NOT be used for high accuracy base mapping such
as property parcel boundaries.
1:100000 scale datasets are recommended for projects that are at the
multi-county or regional level.
1:125000 scale datasets are recommended for projects that are at the
regional or state level or larger.

Vector datasets with no defined scale or accuracy should be
considered suspect. Make sure you are familiar with your data
before using it for projects or analysis. Every effort has been

GeoPlan downloaded this data and exported a subset of the .dbf / Attribute Table.
The subset included:
   ABORTION CLINIC
   DIALYSIS CLINIC
   MEDICAL DOCTOR
   NURSING HOME
   OSTEOPATH
   STATE LABORATORY/CLINIC
   SURGICAL CENTER/WALK-IN CLINIC

The .dbf was then geocoded against a combination address locator, 
derived from parcel centroids and street centerlines, and converted into 
a shapefile with the FGDL Albers HPGN map projection.

Please Note: The following point data represents only Geocoded features, these 
points HAVE NOT been validated via QAQC methods such as aerials or Google Street View. 
Records that have been marked with a FLAG = V are locations that have geocoded to a 
parcel's centroid, based on the parcels physical address.

Next GeoPlan Center performed the following attribute table adjustments.

-Upper cased all records.
-Added DESCRIPT field based on NAME
-Socio-Cultural Effect data fields and definitions were added. 
 FLAG, UPDATE_DAY, NAME, ADDRESS, CITY,
 ZIPCODE, OPERATING, OP_CLASS, FGDLAQDATE.
